VISIT TO PRACTITIONER: not limited

VISIT TO SPECIALITY DOCTORS: 15 visits in case of valid medical grounds.

The plan includes the following specialty doctors: obstetrician-gynecologist, neurologist, urologist, surgeon, ophthalmologist, otorhinolaryngologist, dermatovenerologist, cardiologist, gastroenterologist, endocrinologist.

LAB STUDY (UPON DOCTOR’S REFERRAL):

• Clinical blood test and urinalysis – not limited.
• Biochemical blood test– 15 parameters from the list.
• Hormones – (total Т3, free Т4, TSH) – once a year.
• Serological tests – 5 parameters from the list.
• PCR diagnostics – 5 tests from the list.
• Cytological tests – 1 test from the list.
• Microscopic test – 5 parameters from the list.
• Bacterial tests – once per each test from the list.
• Coagulation profile – 5 tests from the list.

INSTRUMENTATION DIAGNOSTICS (UPON DOCTOR’S REFERRAL):

• USI of abdominal cavity organs, kidneys, bladder, small pelvis – transabdominal (for women), mammary glands, thyroid gland – not more than 3 tests suggested by the doctor.
• X-ray imaging – not more than 2 tests.
• Endoscopic exam – once (esophagogastroduodenoscopy or colonoscopy).
• ECG – twice within the service period.
• Respiratory function – once within the service period.

MEDICINAL MANIPULATIONS (UPON DOCTOR’S REFERRAL):

1. Therapeutic massage – 1 course of 10 sessions provided doctor’s referral and medical grounds 1.5 massage units within the service period.
2. Intramuscular, intravenous injections, (exclusive of the drug cost) – not limited.
3. Course of ENT disease treatment (rinsing of lacunae of tonsils, nasal irrigation) – total of 2 procedures within the service period.

HOUSE CALL:

One visit provided valid medical evidence of a disease; therapist; on business days; during the Clinic’s working hours. Visit within Moscow Ring Road only.

One (or several) of the following symptoms can serve as a ground for house call:

• Fever.
• Respiratory dysfunction (choking, dyspnea, cough, runny nose).
• Asthenia.
• Severe headache.
• Skin rush.
• Other reasons that prevent the patient from visiting the Polyclinic.

ISSUANCE OF MEDICAL DOCUMENTATION:

1. Temporary disability slip.
2. Drug prescriptions (except for privileged).
3. Certificate to swimming pool – once.
4. Health certificate.
5. Abstract of outpatient medical record.

ADDITIONAL: Chiropractor – two visits within the service period.

2. Biochemical blood test – 20 parameters from the list.
3. Hormones – (total Т3, free Т4, TSH) – once a year; from group (LH, FSH, prolactin, progesterone, estradiol) — total of 3 studies.
4. Serological tests – 5 parameters from the list.
5. PCR diagnostics – 10 tests from the list.
6. Cytological tests – 2 tests from the list.
7. Microscopic test – 10 parameters from the list.
8. Bacterial tests – 3 tests from the list.
9. Coagulation profile – 5 tests from the list.
